RISK ASSESSMENT FOR LIFTING OPERATION WITH A LORRY LOADER

  • High wind causes the load to collide with a fixed object
  • Lorry loader becomes unstable

  • Physical injuries, loss of assets due to device & equipment damage, contamination with wastewater, chemicals and dust, unhygienic condition

4. Control Measures to Avoid or Minimise Risk

  • The public excluded from a secure site
  • Establish an effective exclusion zone in conjunction with Principal Contractor
  • All personnel to wear high visibility clothing
  • Ensure the lifting team is fully briefed on the need to keep clear of load during lifting
  • Establish the presence of voids/underground services with the Principal Contractor
  • Assess ground and establish the required size of stabilizer mats
  • Crane lifting Supervisor to check  and verify that mats supplied those specified in Method of Statement
  • Ensure the weight of the load is known and accurate
  • Lorry Loader operator to have valid ALLMI/CPCS card
  • Make sure the lorry loader has been properly maintained and has the current report of thorough examination and evaluation.
  • The tag line to be attached to the load to control rotation
  • Establish an effective exclusion zone in conjunction with Principal Contractor
  • Develop the presence or otherwise of overhead lines. If exists arrange for isolation or position lorry loader boom/load outside minimum safe and secure approach distance
  • The tag line to be used
  • Gloves to be worn
  • All slinging to be completed by Slinger/Signaler with valid ALLMI/CPCS card
  • Make sure that the lorry loader has been appropriately well maintained, has the current report of a thorough examination and that pre-use checks are carried out.
  • Ensure lifting accessories with adequate capacity have been selected, that they are adequately maintained, have the current report of a thorough examination and that pre-use checks are carried out.
  • Inspect load for lost objects before the lift and secure/remove loose items.
  • All personnel to wear hard hats.
  • Sling to be pre-attached to lifting points on the cage roof. 
  • Slinger-signalers to stand on a ladder with the upper half of the body through the trap door to attach/remove the master link.
  • Wind speed to be examined and checked with handheld anemometer by Crane lifting Supervisor before the lift and lifting operation get started. Lift to be aborted if wind speed exceeds 20 mph.
