Control Measures

  1. COMPANY’s employees shall have mandatory training.
  2. Work permit from the client, with all concurring parties' signatures approval, must be obtained before the start of work and displayed at the site
  3. Safety requirements stated on the work permit must be available on-site and strictly followed
  4. Toolbox talk shall be conducted to all personnel on-site & documented before the start of work.
  5. Appropriate PPE shall be worn by all personnel at the site.
  6. Assigned trained dedicated Banks man for the job with reflective jacket.
  7. No water no work policy shall be followed.
  8. Work Schedule according to heat Index
  9. Ear protection shall be worn where required.
  10. A noise meter will be made available at the site.
  11. Maintenance of equipment on regular basis.
  12. Use of Approved/Trained license drivers.
  13. Vehicle movements on site shall be controlled by the bank's man
  14. Appropriate signage to be provided.

2-Activity

Transportation and shifting materials for the scaffolding using various vehicles such as heavy cranes, trailers, trucks, pickups, etc.


Hazards

  1. Failure of equipment /crane /Trailer/ Pickup/ Truck etc.
  2. Road traffic accident during transportation of Tanks shell plates, pipe spools/ supports, and other material.
  3. Body part injury during loading and unloading operation
  4. Pinch point.

Control Measures

  1. Conduct toolbox talk and LMRA before the start of work at the specific location.
  2. Ensure the condition of equipment, is inspected, the sticker is Placed, and the appropriate color code is.
  3. All rigging gear shall be certified.
  4. A synthetic belt or nylon sling shall be used instead of steel wire rope for holding material.
  5. Ensure crane operators and riggers fully understand all work tasks and hazards.
  6. The load shall be secured on the trailer bed to prevent accidental falls.
  7. The Crane operator shall have a third party, certified, and applicable license.
  8. Use proper tagline during loading /offloading material.
  9. Access route shall be surveyed before transporting the Shell Plates, pipes & support material, etc.
  10. Barricade with an appropriate signboard on the effective area. 
  11. Riggers shall be well competent and experienced and third-party certified.
  12. Use leather gloves for hand protection.
  13. Keep away body parts (hand, feet, head, etc.) from the line of fire (LOF).
  14. The suitable length of the trailer shall be arranged according to the length of Shell Plates, pipe spools support, etc.

Control Measures

  1. Conduct ‘TBT’ before the start of the activity.
  2. Prepare, develop and conduct toolbox talk based on JSA and specific job site requirements Obtain PTW, review, and communicate to all involved in the task.
  3. Participate and communicate with area management to ensure no adverse interface impact or hazard.
  4. Install correct warning signage and barricading for the observation by other parties
  5. Follow recommended maximum stacking guidelines.
  6. All scaffold materials shall be stacked and segregated safely and by type.
  7. Stacking and storage guidelines shall be prominently signposted in easy visual display.
  8. Shelving/racking shall be stable and appropriate; pallets when used shall be in good condition without defect.
  9. Pipes and/or tubular components shall be adequately chocked.
  10. All scaffold personnel shall have attended and passed project ‘Manual Handling’ training programs.
  11. Weight and packaging shall be checked and evaluated before handling.
  12. PINCH POINTS shall be identified and hands shall not be placed in the ‘Line of Fire’ between unsecured items or under loads being raised or landed.
  13. Eye protection shall be worn for dust particle injury.
  14. Eyewash facilities shall be provided and First Aid equipment and personnel shall be available
  15. Scaffold personnel shall be trained in the use of fire extinguishers and/or other firefighting equipment (blankets, etc.).
  16. Emergency numbers and fire hazard signage shall be prominently displayed.

  56. Inertia reel to have a valid test certificate and be correctly color-coded.
  57. Follow BS EN requirement, lift to be no more than 2 m vertically.
  58. Ensure ties are placed and are not removed throughout the scaffold's life.
  59. Any adaptions (removal of ties) should be undertaken by advanced scaffolders or under the direct supervision of the foreman.
  60. Communication between the scaffolding supervisor and the end-user supervisor to ensure the scaffold is fit for the purpose intended.
  61. Ensure the correct sequence of erection and that all components meet the BS EN criteria.
  62. Ensure sleeves are used and staggered.
  63. Joints in different bays at each level of the scaffolding to ensure a ‘sound’ construction method.

4- Activity

Confined space entry.

Control Measures

  1. All personnel working in confined spaces must have confined space training.
  2. PRL confined space procedure shall be followed.
  3. Hole watcher is attentive all the time during confined space activity. 
  4. 24V Lighting to be provided.
  5. Easy communication between the hole watcher and supervisor is established all the time.
  6. Ensure that passage and transport route is clear during confined space activities. 
  7. Proper access/agreement should be maintained.
  8. Gas test to be done by Contractor/Sub-Contractor before entering into the confined space.
  9. A trained and qualified individual should be outside the tank with the responsibility of ensuring the safety of authorized entrants (Fabricator, Fitter, Welder e.tc) through the performance pre‐entry briefing, checking and inspecting PPE, equipment, and tools, and performing regular monitoring to ensure person working inside pipe remains safe for entry and occupation.
  10. A written rescue plan that has provisions for conducting a timely rescue of individuals working inside the pipe (Confined Space) should be kept on site. 
  11. Raising the alarm in the event of an emergency the use of the radio/mobile phones supplied to them.
  12. Ensure effective communication systems for entry are tested.
  13. Proper light should be provided less than 25v.
  14. All materials stack proper and category-wise.
  15. Rescue arrangements shall be made while working more than five meters inside a large bore size.
  16. PRL SHE&S Procedure shall be followed.
  17. Close supervision is required while working inside.
  18. Execute the work under close supervision and the presence of a hole watcher.
  19. Proper air supply to be provided or an air mover will be provided.
  20. Proper ventilation shall be there.
  21. Working crew shell is trained for confined space working.

  1. Educate the workforce about Emergency Procedures.

  5. All Workforces shall know the Muster Point location.

6-Activity

Hazards 

Improper, inadequate, and insufficient provision or availability of lighting in and around work immediate areas/ vicinity, creating shadows or poorly lit work areas that could result in unsafe acts and physical injuries taking place


Control Measures

  1. Adequate lighting in the work area and Access/Egress area. 
  2. Emergency direction to muster factor to be illuminated.
  3. Proper cable management
  4. Use clear safety glass for night work for good visibility.
  5. Warning Lights and signboards.
  6. Trained Flagman for equipment movement provided with a signal torch.
  7. Close supervision is required while working in night shift.

Housekeeping


Hazards 

  1. Trips and fall
  2. Sharp Objects
  3. Equipment damage
  4. Control Measures
  5. All waste generated will be contained and disposed of in certain waste skips.
  6. Do now not keep materials/devices in the plant place.
  7. Use a Proper Personal defensive system for dealing with waste.
