  3. Excavation collapse due to unstable ground or failure to shore the excavation.
  4. Accumulation of excavated material too close to the excavation edge leading to collapse.

Risk Control Measures

  1. A work permit shall be provided before work commences.
  2. Provide appropriate PPE and ensure regular induction training and toolbox talks are conducted before starting work. Implement appropriate engineering controls to ensure health and safety. 
  3. Direct supervision of supervisor. Define and provide stable barricades. An exclusion zone should be provided. Safety Warning signs shall be in place, providing designed and approved shoring.
  4. Adequate sloping and benching of sides. Planned/Defined storage of equipment and safe routes for equipment.
  5. Post flagman / Banksman to control vehicle movements.
  6. All equipment is to have reverse alarms. Provision of stand-by pump.
  7. A maintenance program should be implemented.
  8. Ensure Maintenance of equipment will be carried out regularly.
  9. Employ trained, competent, and experienced operators.
  10. Operation manuals are in place. Avoid overloading of material.
  11. Define Obstruction free access and egress. Cover transported material.
  12. All personnel are to use dust masks and goggles.
